How to apply for a Person with Disability Identification Card (PWD-IDC)
What is a PWD-IDC?
According to RA 10747 or the “Rare Diseases Act of the Philippines”, those with rare diseases such as Scleroderma are now considered as a person with disability. This allows them to apply for the Person with Disability Identification Card (PWD-IDC).
As of April 2025, those with PWD-IDC can get a 20% discount on goods and services from establishments such as medicine, medical treatments, transportation fees, and educational fees.
Additionally, this discount also applies to the purchase of products and services for the exclusive use of the person with disabilities.

Submission of Documents
As of April 2025, these are the steps and documents needed for the application of PWD-IDC.
Acquire and fill out the Person with Disability - Application Form (PWD-AF).
You can acquire these forms from:
Municipal and City Social Welfare Offices or Persons with Disability Affairs Offices (PDAO)
Department of Health Offices
National Council on Disability Affairs Offices
Prepare the necessary documents.
The initial application for PWD-IDC is free of charge. Prepare the following documents for the application:
For applicants with cancer or rare diseases, provide a Medical Certificate from the appropriate physician.
School ID or Birth Certificate for children or minors. For those of legal age, one valid Government-issued ID.
Two recent “1×1” ID pictures with signatures or thumb marks at the back of the picture.

Submit documents.
Submit the required documents to the Person with Disability Office (PDAO), Barangay Office or the equivalent in your city.
Once the office has screened and approved your submission, they will provide you with your new PWD-IDC.

How to check for the validity
of PWD-IDC?
Before using the PWD-IDC to avail of its benefits, it is recommended to first check for the validity of the ID.
As of April 2025, you can visit the Department of Health’s Philippine Registry for Persons with Disability and input the ID number on your PWD-IDC. If the ID is valid, it should provide the same information that is present on the card.
